Blood is taken from the mother, usually between the fifteenth and twentieth week, and tested for alpha-fetoprotein, hCG and unconjugated oestriol. Those levels, read together with the mother's age and the exact stage of pregnancy, produce a statistical risk figure for Down syndrome and for defects of the spine and brain. It screens; it does not diagnose.
Doctors order it for routine antenatal screening in the second trimester, particularly where the mother is older or an earlier scan raised a question.
