Two substances that come from the pregnancy, free beta hCG and PAPP-A, are measured in the mother's blood at roughly 11 to 14 weeks. Those levels are read along with the mother's age and the nuchal translucency measured on the scan, and a risk figure is worked out. This is a screening test, so it gives a probability and not a diagnosis.

Doctors order it for routine antenatal screening in the first trimester, and particularly where the mother is older or there is a family history of a chromosomal condition.
