ESR does not name a disease. It rises when there is inflammation anywhere in the body — infection, arthritis, and many other causes.
It is almost always read alongside a CBC and your symptoms, never on its own.

- The tube has to stand straight for a full hour
- A Westergren tube tilted even a few degrees lets red cells slide down the side, and the reading comes out falsely high. Ours stand in a levelled rack, away from vibration, and a timer decides when the hour is up, so the reading is taken at sixty minutes and not at seventy.
- Room temperature changes the number
- ESR rises when the sample is warm and falls when it is cold, so the same blood can give two different figures depending on where the rack was kept. The rack has one fixed place at bench temperature, away from the window, and a sample from the fridge is brought back to room temperature before it is run.
- ESR on its own does not say much
- A raised ESR tells you there is inflammation in the body. It does not tell you where. Anaemia by itself pushes the value up, so a high ESR in a woman with low haemoglobin may be about her iron. One lavender tube is drawn, and the complete blood count runs from the same sample.
- The same method every time you repeat it
- ESR is usually repeated over weeks to see whether treatment is working, and that comparison only means something if the method has not changed in between. We run Westergren for every sample, which is the method the printed reference ranges are written for.
- Rs 200, and no fasting needed
- You need not come empty stomach for an ESR, so it does not have to be a separate morning trip. The charge is Rs 200, with nothing extra for the report on WhatsApp. The full price list is published.
