The picture is taken with the beam passing from the front of the knee through to the back, showing the lower end of the thigh bone, the upper end of the shin bone and the gap between them. That gap is where the cushioning cartilage sits, so its width says a good deal about wear. The kneecap appears as a shadow lying over the joint.
