Ordered before surgery, for liver disease, and to monitor patients on blood thinners like warfarin. The INR is a standardised number so the result means the same thing in any laboratory.
The blue tube has to be filled exactly to the line, which is why this sample is drawn carefully and never topped up afterwards.

- The blue tube is filled to the mark or drawn again
- Prothrombin time depends on a fixed nine-to-one ratio of blood to citrate. A short-filled tube carries too much citrate for the blood in it, and reports a longer time and a higher INR than the patient has. On warfarin, that can mean a dose cut nobody needed. If the tube stops short of the line, we draw again.
- A difficult draw shows up as a wrong result
- Fishing for a vein activates clotting inside the syringe, and a small clot in the tube cannot be seen once the plasma is separated. The number then comes out wrong with nothing on the report to warn you. A slow or clotted draw is repeated here rather than run.
- The INR is tied to the reagent's ISI
- An INR is meant to read the same here as at the lab you used last month. That only holds when the ISI of the actual reagent lot is entered into the analyser and the calibration is current. Our laboratory is NABL accredited to ISO 15189, and the ISI is updated with each new lot.
- Before surgery it is rarely the only test asked
- Surgeons usually ask for PT along with an ECG and a chest X-ray, and sometimes a physician's opinion on the whole set. All of that is available in this building: 78 X-ray views on the list, ECG, 2D Echo, and four consultants who see patients here.
- Rs 800, with the INR on the same report
- One charge of Rs 800 covers the prothrombin time in seconds, the control value and the calculated INR. There is no separate fee for the report or for sending it to your WhatsApp. All 286 priced items are on the public price list.
