The sinuses are hollow chambers in the skull that normally hold air and so look black on a film. When they fill with fluid or the lining swells, they turn grey or white, and the difference between one side and the other becomes easy to see. The picture is taken sitting upright with the chin raised, so any fluid settles and shows a level.

Doctors order it for a long-standing blocked nose, facial pain and heaviness, headache over the forehead or cheeks, and colds that keep coming back.
