Thyroid peroxidase is an enzyme the gland needs in order to make its hormones. Antibodies against it are the commonest marker of autoimmune thyroid disease and are found in most people with Hashimoto's thyroiditis. How high the antibody level goes does not track how underactive or overactive the gland has become.

Doctors order it for a raised TSH, goitre, unexplained tiredness or weight change, and thyroid assessment in pregnancy.
