Dihydrotestosterone is formed from testosterone inside the skin, the hair follicles and the prostate by an enzyme called 5-alpha reductase. Its level tells the doctor how much of that conversion is going on, which a testosterone reading on its own cannot show. It is done on a plain blood sample drawn from the arm.

Doctors order it for male-pattern hair loss, unwanted hair growth in women, prostate enlargement, and to follow treatment with medicines that block the enzyme.
