Most of the T3 in the blood is stuck to carrier proteins and cannot act on the tissues. The free portion is the part actually available to work, so it reflects thyroid activity even when protein levels are shifted, as they are in pregnancy or with certain medicines. A plain blood sample from the arm is all that is required.

Doctors order it for suspected overactive thyroid, for palpitations and weight loss, and to follow patients already on thyroid treatment.
