Thyroxine moves through the blood largely attached to carrier proteins, and only the small unbound share enters the tissues and does any work. That unbound share is what this test measures. It is commonly read next to TSH, since the two shift in opposite directions when the gland is working too little or too much.

Doctors order it for suspected underactive or overactive thyroid, for follow-up of someone on thyroxine tablets, and during pregnancy where thyroid disease is known.
