TSH comes from the pituitary gland and rises or falls as it pushes the thyroid to work, while free T3 and free T4 are the hormones the thyroid itself releases. Measuring all of them at once lets the doctor separate a problem in the thyroid gland from one in the pituitary. It is the usual panel when the thyroid is being assessed for the first time.

Doctors order it for tiredness, weight change, hair fall or irregular periods, for a swelling in the neck, and to review the dose of thyroid medicine.
