Standing is the point of it. Gas rises and liquid settles, so a loop of bowel that is blocked shows a stepped line of air sitting over fluid, and air that has escaped from a perforation collects as a thin dark crescent under the diaphragm. Lying down, neither of these findings would be visible on the film.

Doctors order it for sudden severe abdominal pain, vomiting with a swollen abdomen, constipation with no passage of gas, and suspected obstruction or perforation.
