The X-ray is taken from the side of the face, so the space behind the nose appears as a dark airway on the film. Anything filling that space, most often enlarged adenoids in a child, narrows the airway and can be measured against the bone behind it. This is usually the first picture taken when a child breathes through the mouth or snores.

Doctors order it for mouth breathing, snoring at night, a blocked nose in children, repeated ear trouble, and a change in the sound of the voice.
