The direct part uses your own red cells and shows whether antibodies are already stuck to their surface. The indirect part uses the serum and shows whether antibodies are floating in it that would go after red cells given to you later. Both are run in this single request, since they answer two sides of the same question about red cells being destroyed.

Doctors order it for anaemia thought to be from red cells breaking down, for jaundice in a newborn, and during antenatal care and cross-matching.
