TSH is made by the pituitary gland at the base of the brain and tells the thyroid in the neck how much hormone to produce. The pituitary pushes TSH up when the thyroid is underactive and drops it when the thyroid is producing too much, so the level moves opposite to thyroid activity. It is usually the first thyroid test done.

Doctors order it for tiredness, weight change, hair fall, irregular periods, a swelling in the neck, and to adjust the dose of thyroid medication.
