You sit or lie with the knee bent and the X-ray is aimed down along the front of the joint, as though looking along a skyline. This is the only view that shows the underside of the kneecap and how well it is riding in the groove at the end of the thigh bone. Ordinary front and side pictures cannot show that surface at all.

Doctors order it for pain behind the kneecap, a kneecap that slips out of place, grinding while climbing stairs, and after a direct blow to the front of the knee.
